A patient's health status may change at any time during a home care episode. He may need emergent care or transfer to an inpatient setting. Lessons in this module introduce OASIS items M0830 through M0906. Some data items provide information about any emergent care services (including the type and reason), type of inpatient facility a patient may be transferred to, and reason for admission. Others relate to discharge of a patient from the agency for reasons other than transfer to an inpatient facility, and report the discharge disposition and any supportive assistance the patient will receive after discharge from the agency. One item reports the date of the last home visit.
