Consider the learning needs of a patient who is depressed. Sadness, feelings of hopelessness, or lack of concentration can prevent even the simplest of instructions or health teaching from having any meaning. As a result, any desired change in health care behavior is not possible or is not easily or promptly achieved. Is there the possibility you will need to teach caregivers, and if so, what and how much, and how long will it take? Because this item doesn't provide a place to record a complete description of those depressive feelings reported or observed, be sure to document such information in the clinical record in the appropriate places.
