Response options are arranged in order of increasing frequency from none of the time to all of the time with a choice of NA if the patient is non-responsive. Select only one response. NA is the appropriate response if the patient is unable to respond, that is, is non-responsive for whatever reason. A patient who refuses to answer, but whom you observe to be anxious during the visit, is not non-responsive.
