Clinicians use both observation and assessment strategies to accurately respond to these items. Remember OASIS is not a survey or questionnaire. Respiratory assessment is done throughout the visit. Most of what you assess contributes to your decisions about the care plan. Most of your findings are recorded elsewhere, not just marking the OASIS item. There's no place to record all the details, and specifics should be made in clinical notes as well.
