At every point in the assessment you have a chance to evaluate verbal speech. Listen carefully as the patient tells about caregiver assistance, bathing, meal preparation, and how he manages getting to doctor appointments. Are words clear? In understandable sentences or phrases? Is he trying to tell you something familiar but can't without prompting or assistance? Is he struggling to produce a word, sentence, or phrase? Don't hesitate to confirm your findings with family or caregivers to be certain this is usual. Since the item doesn't allow space to describe details, do make a note of these in other documentation.
