Even when a patient volunteers this information freely, clarify or confirm the response with the primary caregiver or others. For example, patients who are confused or forgetful can be unclear or unaware of the specifics of assistance. When assistance comes from different sources, she may not have a clear sense of who takes lead responsibility. Such situations may require later telephone follow-up with others (e.g., referral source, apartment manager, family member, etc.)
