Your initial responses, based on what the patient tells you early in the visit, may change as you observe functional status and talk more with the patient about what assistance they need and what is actually provided. For example, if meds are neatly arranged in pillboxes for specific days and times, ask who does the medication set-up. If she responds, “Oh, my neighbor helps me with that,” ask whether the neighbor provides any other assistance.
