Historically, agency assessment forms attempted to record a myriad of findings from the home visit. The differences in both clinician assessment skills and clinical documentation meant that the information rarely provided definitive information about an agency's quality of care. Uniformly defined patient characteristics are essential to accurately report the effectiveness of an agency's care through patient outcomes.
